Special Circumstances Form (PGR)

The Special Circumstances Form (PGR) is a multi-purpose form for MPhil or PhD students only, which may be used to report a wide range of short-term absences, including:

• Absence due to illness
• Personal/Family problems
• Difficult events or serious incidents, such as crime or bereavement.

The Special Circumstances Form (PGR) should only be submitted for absences of less than 30 days, and when applying for a Leave of Absence would not be appropriate.

Students are advised to contact their Supervisor or Personal Tutor before completing the form. Once the form has been completed, it should be returned to the appropriate Supervisor or Personal Tutor.
