Change of Address

1. Inform the Health Service of any change of address or telephone number including mobile numbers. You can do this on-line at
www.shef.ac.uk/health/information/address.html

2. A doctor is not obliged to visit you if you have not informed the practice of your change of address.

3. The Health Service´s records are separate from the University´s. So, if you change your address on the University record, you must inform the Health Service separately.
