Registration University Health Service

All new students should attend a medical interview with staff of the University Health Service.

1. To register with the University Health Service (or any other practice) you must:

  1. Complete on-line registration pages and medical questionnaire (see www.shef.ac.uk/health)

    For guidance on how to complete the medical questionnaire (see “Registration: The Essential Guide”).
  2. Complete form GMS1 – this will take place when you attend your registration interview

    • bring with you either your current medical card or your NHS number and current doctor’s name and address

    • international students who have not previously been registered with the NHS, must tell us the date you first arrived in the UK.

  3. be interviewed by a doctor or nurse (if appropriate).
  4. Have a fuller medical investigation (subsequently by appointment) if the doctor advises this.

2. Females (students or dependants) aged 25 or over: if you are/have been sexually active and have had a cervical smear (Pap test) in your own country/home area bring a report of this medical test when registering with the Health Service. This will allow us to follow up any abnormalities for you.
It is routine to ask all women over the age of 25 to attend for a test but, as it is not always indicated, you can ask for it to be deferred. If you wish to have the test done, please make an appointment with a nurse.
