Claim form for Wellcome Trust open access fees

Please complete all sections of the form to ensure your claim can be processed promptly. Please also email or post the other requested documents immediately. Your claim cannot be processed without them.

The completed form will be submitted directly to the University Library. If your claim is in order, you will receive an email authorising you to proceed.
