Self-Assessment Form (SAF)

The information provided on the SAF is for our information only. However, if there adjustments that need to be made in advance of you studying at the University, we will contact you to ask for your consent to share information with your academic department or other relevant service.

Please be aware that submitting a SAF does not mean that support will automatically be put in place for you. In order to ensure that this happens, upon arrival at university you will need to take further steps which we will inform you about in future correspondence.

In order to access support during your time here you will need to provide our service with documentary evidence of your disability. Further information on appropriate evidence can be found in our handbook on pages 7-8. DDSS Handbook

Self Assessment of Needs

Please indicate which of the following types of support you are likely to require with a tick in the relevant box. You may find our handbook useful to help you complete this section. (Please refer to the "Downloads" box for more details.) The provision of some of the types of support listed below will depend upon (a) whether you are awarded Disabled Students’ Allowances by your funding body and (b) the resources available at the University of Sheffield.

Emergency Evacuation

Will you need assistance to evacuate a building in an emergency? (required)
Will you have difficulty hearing the fire alarm within the teaching and learning areas? (required)

Accessing Taught Sessions

Do you have mobility difficulties?
Are you able to use the stairs independently?
Are you a wheelchair user?
Do you require wheelchair accessible teaching environment?

Personal and Recreational Support

If you are an overseas student, you should be aware that we are unable to cover any costs related to personal care or recreational support which you require during your time here as a result of your impairment. Personal care support includes assistance with shopping, cooking, cleaning, washing and toileting. Recreational support centres upon offering assistance to enable an individual to participate in non-academic recreational activities such as visiting friends, attending Student Union society events or going to the cinema. If you are likely to require assistance in order to undertake these kinds of tasks or activities during your time at the University you need to ensure in advance that you have sufficient personal funding to cover the cost of such support.

Accommodation Requirements

If you have any specific accommodation requirements such as wheelchair accessible room, accessible alarms for hearing impairment, adapted space for specialist equipment, it is important that you contact The Accommodation Office as early as possible in the applications procedure.
