Telephone fault report form
Please note that the location for reporting telephony related faults has moved. Faults can be submitted through the IT Service Desk Self Service area within MUSE. To do this complete the following steps:
- Log in to your MUSE account at www.sheffield.ac.uk
- Click on 'My Services' at the top of the screen and then select 'IT Services (Self Service)' from the drop down options.
- Click on 'Service Requests (forms)'
- Scroll down to find and then click on 'Telephone fault report form' to access the new form.
Please note that this form is only for reporting faults and these should be submitted by the relevant Telephone Liaison Officer (TLO).
