University maternity assistance form

Before completing this form please contact the Student Advice Centre to enquire whether you are entitled to any government financial assistance during your pregnancy, such as Statutory Maternity Pay or Maternity Assistance:

  • If you are not eligible for assistance from another organisation/government, please complete this form at least two months before commencement of your maternity leave.
  • The maternity leave needs to be already approved and entered in your record (you need to request a Leave of Absence for medical reasons and submit it to your department alongside the MATB1 form you have received from your midwife).
  • In order for the University to make any payments, you need to enter your UK bank account details following these instructions.
  • Ask the Student Advice Centre Officer to send us a confirmation of your ineligibility for other assistance to pgr-scholarships@sheffield.ac.uk. Depending on the funds available, we may be able to offer you a one-off payment of £1,500.  It is expected that the minimum period of leave will be 6 weeks.
