How to request to view your medical records.

At the moment we’re not offering printed copies of medical information. We can email the information to you, or allow you online access to your medical records.

You can request to view your medical record via the following ways:


Online

Registering with an online service such as The NHS App, PatientAccess or Evergreen Life will enable you to view your medical record online, and some services will allow you to print or download any part of your record that you wish. 

If you’re not already signed up with one of these services please visit the online services page for information on how to do so.

Once you are able to login to one of these services we can activate access to your record, starting on a date chosen by you.


Paper copy

We are not currently producing paper copies of your records. We can email a copy of your medical record to a verified email address. This will be password protected, and your password will be sent to you via SMS text.

You should allow up to 28 days for your request to be processed. If you have not been contacted regarding receiving an emailed copy of your medical record within this time, contact the practice on +44 114 222 2100.


Immunisation/vaccination history

Contact our Reception team on +44 114 222 2100 who will be happy to email you a copy of your immunisations or set you up with access so that you may view them online.


Accessing your medical record

Follow the link below to download the Access to Medical Records form. This can be completed electronically, or printed and completed by hand. 

Once complete, email the form to syicb-sheffield.uhs.dq@nhs.net

Access to Medical Records form (PDF, 156 KB)Once we've received your completed form and processed the request we'll contact you to let you know when your record is ready to be accessed/collected. You should allow up to 28 days for your request to be processed.
