Fit / sick notes
When and how to request fit notes.
Fit notes only apply to employment. If a medical condition is affecting your studies at the University of Sheffield you will need to apply for Extenuating Circumstances instead.
Before requesting a fit note, please see the NHS national guidance.
If you require a fit note and have been seen by a UHS healthcare professional about the issue you should email Syicb-sheffield.universityhealthservice@nhs.net. Include the name of the healthcare professional you saw and when you saw them if possible. We may be able to issue a fit note without you needing another appointment.
If you require a fit note but have not been seen by a UHS healthcare professional about the issue you should contact reception to arrange an appointment.
