Register with the University Health Service
Registering as a patient at the University Health Service is a simple 2 step process:
- Complete our on-line pre-registration medical questionnaire.
- We now need to meet you and have you complete the NHS Registration Form (GMS1). Please visit the UHS reception or call 0114 222 2100.
If you do not need to be seen urgently, please allow 2 working days before you contact us. This will give us time to process your information.
Who can register with the University Health Service?
- All University of Sheffield students who live in Sheffield can register with UHS.
- Dependents (the spouse or child) of a student can also register if they live within our practice boundary.
From the 1st April 2015, all patients at the University Health Service will be given a named accountable GP. They will be responsible for your overall care at the Practice. You will still be able to book appointments with any doctor at the University Health Service that you choose. If you wish to know who your named GP is or if you have a preference please contact reception.
To complete your registration, you will need to print out and complete the GMS1 form. Please do not complete this form until you have completed your pre-registration.
